Whether the injury is a result of an automobile accident, a sports mishap, an altercation or a bad fall, the severity and type of injury will determine the treatment necessary. Chipped, dislodged or avulsed teeth and root fractures account for the majority of all dental trauma.

If you have been referred to our offices, you will need to fill out a patient information form. If you choose not to complete the form online, you can complete it when you first visit our office.
